Unplan Extubation Prevention

Identify an RN or RCP who is caring for an intubated patient on a ventilator. Observe a high risk intervention as defined on back of card. Ensure all elements are completed .

Observe that an Airway Guardian” is used to keep ET tube secure during high risk interventions (see back of card for details).

· (2) clinical people are present.  Observe the dedicated Airway Guardian (RN or RCP) holding and anchoring the ET   

                   tube to avoid dislodgement.

· Always request an Airway Guardian for high risk interventions.

¨ Patient transport

¨ High risk patient position changes, transfers to gurneys, parents lap, CT or MRI scanner procedure

¨ Invasive bedside procedures such as LPs, bedside surgeries, etc.

¨ X-rays, weights, linen changes, baths

¨ Repositioning of Airway Alert precaution patients or HFOV patients

 



Airway Guide is posted at the head of the bed.

· Patients with a prior unplanned extubation or difficult airway are identified  by checking the Airway Alert Precaution box on      the  Airway guide.

· Verify ET tube securement point is accurate. 

 



Have the RN/RCP demonstrate how to assess ET tube security and verify securement point corresponds with Airway Guide.

· Manually grasp the ET tube and check for mobility.

· If at any time the ET tube security is  questionable – stop and re-tape!
