Inpatient Psych Safety

Identify a staff member who is sitting for a patient. Ensure the following elements are completed and documented (where applicable).

Ask the staff member, was an appropriate handoff given by previous sitter?
Ask the staff member, did the RN give an update?
Does the staff member know what precautions their patient is on and can they explain what the precautions mean?
Is the staff member following the precautions correctly?
Check documentation with the staff. Ensure the following is complete. Check any MISSING elements below.
Environmental rounds - At shift change was there a 2 person environment check completed?

 
Environmental rounds - Was the in depth room check completed?

 
Environmental rounds - For patient's sitter - Is the patient's room door closed and locked? If you are a float or floor BHT are all unoccupied patient room doors closed and locked?
Environmental rounds - Are dayroom and/or classroom doors closed and locked if not in use? If in use, is there staff in each location so patients are not left unattended in public areas?
