WATCHER

Identify a patient a Level 2 or 3 WATCHER alert. Ensure the following elements are completed within 1 hour of positive screen.

Has everyone been notified?

Ask the nurse about the following notifications:   

· Have the proper people been notified? See back of card

· Is the nurse concerned? Do they feel this is an appropriate fire?

· Show the RN why the alert fired. Review the WATCHER parameters

 
Assess the patient

COMPLETED AT THE PATIENT SIDE:

____  Check ABCs, ask  if theses interventions have been considered or discussed 

· Does patient appear to be toxic?

· Is oxygen needed?

· What are current vital signs, I&Os

· How does the pt look? Color, circulation, respiratory distress?

Proper interventions concidered

· Sepsis Order Set utilization

· IV Access and labs - Suggestions: Bolus, Blood Culture, VBG, lactic acid, blood glucose, CMP, Procalcitonin

· Antibiotics? Do NOT delay antibiotics for cultures

· Vital sign frequency - an order including parameters

· When will the team reassess?
