  • Verify medication preparation is for the right route with medication order or eMAR
  •  Check for IV compatibility


Right Time/Frequency

  • Verify time/frequency with med order or eMAR and previous administration

· Ensure patient is wearing ID band

· Verify 2 patient IDs on med label with those on patient’s ID band

· Scan patient ID band  with  KBMA device

 



Right Dose/Route

· Scan med with KBMA device

· For IV medications, pump library is in use

· Rate and medication entered are correct per label

· For IV medications, trace the IV line; bag/syringe-to pump-to patient

· Ensure correct line(s) are open and not clamped 

 



UPON LEAVING PATIENT SIDE: Right Documentation

Complete the task within KBMA or document on the eMAR*  

* Bypassing the KBMA scan is highly discouraged.  Reasons may include:  emergencies or home medications.

 

Printing multiple patient identification band labels is prohibited unless a replacement band is needed; extra patient identification bands on the unit can lead to unsafe workaround and potential patient identification errors.

 



UPON LEAVING PATIENT SIDE: Ensure Medication Security

No visible unsecured medications at bedside/room

Bedside cart checked and secured/locked
