Has the highest degree of mobility and/or ambulation been encouraged for the patient at least 3 or more times a day?

Encourage highest degree of mobility/ambulation for patients.  Ideally at least 50 feet or more.  If baseline is non-ambulatory or neonate/infant perform passive range of motion, supported sitting in bed, caregiver holding, or dependent lifts into tumbleform chair (1 hour each session) at least 3 times per day.


Is the patient wearing SCDs while in bed?

All patients that have at least 1 VTE risk factor or altered mobility should have SCDs on when in bed.
Altered mobility is the temporary inability to ambulate freely (Braden QD Mobility Score 1-2).
If SCDs are contraindicated, comment below with details.
