Are two licensed clinicians present for securing, repositioning, and/or manipulating endotracheal tubes?

Was the endotracheal tube at the correct anatomic landmark?

  • Validate the ETT is secured by manually grasping the ETT and checking for mobility.
  • Neonates (0-30 days post 40 weeks) secured at lip, otherwise at gum/teeth.


Was the correct securement method used on the endotracheal tube?

  • Tube securement device per manufacturer's recommendations or ETT tape


Were 2 licensed clinicians present during high risk situations?

  • Airway guardian must be dedicated to holding the tube during movement and repositioning.
  • High Risk Situations include at least (use local ACA data to append list):
    • Bedside imaging procedures
    • Bedside invasive procedures
    • Kangaroo care/parent holding
    • Routine repositioning
    • Switching beds
    • Early mobility
    • Linen changes, baths, weights
    • Patient transport
